Funding Acknowledgements
The financial support of the Australian National Health and Medical Research Council project Grant 628527 is acknowledged as is the technical support of the mechanical workshop at Peter MacCallum Cancer Centre. Thanks also to the TROG QA team for reminders and facilitating the plan review. Without the co-operation and support of participating centres the study would not have been possible.
